CPAFH Certificants

Individuals who attain the Certified Professional in Age-Friendly Healthcare (CPAFH) credential represent a group of committed professionals from across healthcare who are determined to advance the age-friendly healthcare field.

The Certified Professional in Age-Friendly Healthcare (CPAFH)™ credential distinguishes healthcare professionals who meet the competency requirements in the areas of Age-Friendly healthcare, and who demonstrate the ability to apply this knowledge to deliver high-quality, person-centered care rooted in the 4Ms Framework and tailored to the goals, preferences, and needs of older adults. Candidates may include Physicians, Nurses, and Pharmacists, Social Workers, Case Managers, and Care Coordinators, Physical, Occupational, and Speech Therapists, Rehabilitation Professionals, Health System Leaders, and other professionals with the requisite background. These professionals may work in a variety of settings, including but not limited to hospitals, health systems, home settings, ambulatory and other outpatient settings, academia, the community, and service providers to the healthcare industry. The CPAFH is open to candidates from the United States as well as practitioners outside of the US.

Those attaining the CPAFH designation represent a group of committed professionals from across healthcare who are determined to advance the field of patient safety. Since the launch of the certification in 2025, more than 100 Age-Friendly professionals have earned the CPAFH credential, representing 31 United States and 5 other countries. Advancing safer care for all requires a commitment to spreading Age-Friendly healthcare expertise across demographics and disciplines.

Congratulations to all candidates who have passed the Certified Professional in Age-Friendly Healthcare (CPAFH ) examination to date. The Certified Professional in Age-Friendly Healthcare credential attests to your professional comp​etency in age-friendly healthcare science and application. The credential is valid for three years, after which time CPAFH Recertification is required.

Please note: All new CPAFH certificants will be listed during the month following your exam pass date. Certificants may opt out of public listing by request, but their status may still be verified by contacting the certification office (certification@ihi.org).
